The reporter’s words

Serious fall hazard: exposed concrete chunks under spinning teeter-totter. Bark needs backfill and/or plastic matting needs to be placed over it, similar to neighboring merry go round and higher-arched spinning see saw. Entrapment hazard: broken plastic seat cover caused my toddler's shirt to get stuck while sliding off to the lowered ground level. This could be a serious hazard for a toggle-trap on a hooded jumper
